
SEYMOUR, Ind. – Schneck Primary Care recently received Patient-Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance for its efforts to provide patients with a personalized and highly coordinated healthcare experience.

The patient-centered medical home model is a way of organizing primary care that emphasizes coordination and communication to transform primary care into “patient-friendly” medical care. Medical homes can lead to higher quality care and lower costs.
